LETTER OF INFORMED CONSENT FOR TRANSPORTATION FOR FIRST BAPTIST CHURCH, GUELPH CHILD AND YOUTH ACTIVITIES

This letter will cover the period from September 1, 2025-August 31, 2026 for transportation as needed.  Consent can be rescinded at any point during the above period in writing to the church office.

While every precaution is taken for the safety and good health of everyone, some activities including transportation, carry with them the inherent risk of personal injury.  Your permission, as the legal guardian, is required to provide this transportation.  Please carefully read the following information and consent form.  If you are in agreement, please indicate your agreement below.

I give permission for my child/charge, hereafter "child", to be transported in a motor vehicle driven by the individual identified to an event at the specified location and date indicated.  I understand that my child is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follw the directions of the driver and/or other adult volunteers.

I have read, understood and discussed with my child that:

1.  They will be traveling in a motor vehicle driven by an adult and they are to wear their seat belt while traveling;

2.  They are expected to respect each other, the vehicles they ride in, and the people they travel with during the trip;

3.  They are to remain in their seats and not to be disruptive to the driver of the vehicle;

4.  Riding in a motor vehicle my result in personal injuries or death from accidents, collisions or acts by riders, other drivers, or objects.

 

I recognize that by participating in an FBCG activity, involving motor vehicle transportation, my child may risk personal injury or permanent loss.  I hereby attest and verify that I have been advised of the potential risks, that I have full knowledge of the risks involved in this activity, and that I assume any expenses that may be incurred in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expenses.

In the event that your child required medical treatment, we will attempt to reach you at the phone numbers above.  If we cannot reach you, I , the parent or guardian named below, authorize the supervising Youth Leader, to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment, or procedures for the participant named above.

I, named below, expect that in the case where medical assistance is needed, the representative of First Baptist Church, Guelph, would make every effort to contact parents before medical treatment is authorized, when possible.

I, named below, undertake and indemnify, and hold blameless First Baptist Church, Guelph, its personnel, and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of First Baptist Church, Guelph, as well as any medical treatment authorized by the supervising individuals representing First Baptist Church, Guelph.  This consent and authorization is effective only when participating in or traveling to or from event of First Baptist Church, Guelph.
